Referral to a Psychiatrist

A referral letter from your GP or Medical Specialist should outline the following details:
Doctor’s name and provider details
Patient's Name, Date of Birth, Residential Address, Phone contact, Medicare Number
Where appropriate Parent/Carer/Case Worker name & contact details
Detailed description of the patient's history, condition/issues, current medication, investigations and any other relevant information

Note no mental health treatment plan is required by the Psychiatrists.

The Psychiatrist may refer a patient to one of our Credentialed Mental Health Nurses to provide comprehensive outreach care. This may include psychiatric medication education, monitoring program efficacy and psychological interventions.

Referral to a Psychologist

A referral letter should outline the following details:
Doctor’s name and provider details
Patient's Name, Date of Birth, Residential Address, Phone contact, Medicare Number
Where appropriate Parent/Carer/Case Worker name & contact details
Detailed description of the patient's history, condition/issues, current medication, investigations and any other relevant information

A Psychologist does require a Mental Health Treatment Plan.

Psychological services are also available through self-referral or private arrangements.

At Mind@Home Psychiatry we invite referrals from General Practitioners and Medical Specialists.

To streamline this process, we ask that referrals to our Psychiatrists are Faxed to 03 5331 4547 or mailed to Mind@Home Psychiatry, 613 Mair Street Ballarat Vic 3350. Alternatively a referral can be delivered directly to the Clinic.
